Optimally, the first line of action is to identify the poisoning substance and call a regional poison control center for direction. Activated charcoal use is often preferred to ipecac syrup as the first line of treatment. Substances adhere to its surface and can be excreted. It does not work for substances in the PHAILS acronym: Pesticides, Hydrocarbons, Alcohol-acids-alkalis, Iron preparations, Lithium, and Solvents. Ipecac syrup as first-line treatment is limited to in-home use with alert children who are found immediately after ingesting a known, noncorrosive substance. It is effective if given within one hour, but it should not cause delay of activated charcoal therapy for non-corrosive substances adsorbed by activated charcoal. (DeBoer S.
