The SNAP Health Center IHP component allows you to create and formalize a plan of support for students with complex health care needs. The IHP template tool makes it easy to produce templates for any health condition. From these templates, you can customize individualized plans for any or all students.

The SNAP IHP module allows you to select interventions and goals based upon the results of your assessments and add them to the student’s custom IHP. Include “related-to” factors, assessment results and update student progress with only a few clicks of your mouse. Extensive libraries give you the power to craft your own IHP master plans. You also have the freedom to exchange and share these plans with other nurses in your district by using the import/export feature.

Can only be purchased in combination with SNAP Health Center Suite. Cannot be purchased as a seperate component.
