For distance visual acuity, this pediatric combination eye chart features LEA Symbols with 2 lines (one for each eye) of linear-spaced optotypes in size equivalents of: 20/40, 20/32, 20/25, and 20/20. Testing distance is 10 feet.

Near visual acuity screening features LEA Symbols with 2 lines (one for each eye) in equivalents of 20/200, 20/40, and 20/32 for critical line screening. The near vision portion also features 2 columns of proportionally spaced optotypes in size equivalents of 20/50, 20/40, 20/32, 20/25, 20/20, and 20/16 for threshold screening. Testing distance for near visual acuity is 16 inches.
