The Utah State Vision Kit – Distance and Near was created through a collaboration between the Utah Department of Health and School Health Corporation. These kits follow 2019 Utah State vision screening requirements.
This kit contains two tiers of testing – one tier for distance testing and one tier for near vision testing.
