WELNET® Module – Survey

SURVEY MODULE provides surveys for students, teachers, and community partners. Included are commonly used surveys such as the Youth Risk Behavior Survey, Student Physical Activity, and Nutrition Survey. Users may also design their own questions and surveys based on their unique needs. Aggregated survey results available for teachers and administrators.

About WELNET:

WELNET is a comprehensive software solution for teachers to gather fitness and health data from students and to promote student learning.
